Primary Care Nurse Practitioner Ajax
BayWest Medical Centre, Ajax
Deliver full-scope primary care in Ajax as a Nurse Practitioner, using health assessment and diagnosis skills on-site at BayWest Medical Centre.
This full-time Nurse Practitioner role serves patients across the lifespan in a community primary care clinic. You will manage acute and chronic conditions through consultations, diagnostic testing, prescribing, and evidence-based care plans. Preventive care and coordinated practice remain central.
Key Responsibilities:
Conduct patient consultations and comprehensive health assessments
Diagnose and manage acute and chronic conditions
Order and interpret diagnostic tests
Prescribe medications and develop evidence-based care plans
Provide vaccinations, education, counseling, and preventive care
Requirements:
Current Ontario Nurse Practitioner license in good standing
Primary care or family/all-ages designation
Master’s degree in Nursing or equivalent
Clinical experience with assessment, diagnosis, prescribing, and condition management
Proficiency with EMR systems and accurate documentation
