Registered Practical Nurse Long Term Care

Deliver person-centred nursing care as a Registered Practical Nurse with Extendicare, applying provincial standards in a part-time setting. Support residents through planned, goal-directed care.
Reporting to the Director of Care or Director of Health and Wellness, the Registered Practical Nurse provides resident-focused clinical care. This position works within Extendicare’s Professional Practice Model and interprofessional team. Responsibilities include assessments, care planning, medication administration, documentation, education, and emergency response.
Key Responsibilities:
Provide direct resident care and administer medications and treatments
Complete assessments, care plans, service plans, and documentation
Evaluate care effectiveness and update plans when required
Coordinate resident assignments for timely nursing service delivery
Respond to significant condition changes and medical emergencies
Requirements:
Registered Practical Nurse qualification
Practice according to provincial standards of care
Work effectively within an interprofessional care team
Follow regulatory and Extendicare policies and procedures
Demonstrate ethics, morality, and integrity
Bring clinical assessment, medication administration, and person-centred nursing skills to Extendicare’s resident care team part time.
