Remote Primary Care Nurse Practitioner

Support families and communities remotely through primary care, chronic disease management, and follow-up services for patients of all ages.
As a Nurse Practitioner, you will provide virtual healthcare supporting Newfoundland and Labrador plus another licensed province. The position requires experience with all-ages primary care, virtual care platforms, and EMRs. Care areas include Family Practice, Community Health, women’s health, and preventative care.
Key Responsibilities:

Deliver longitudinal primary care virtually
Provide chronic disease support for patients
Complete follow-up care for ongoing needs
Build trusting patient relationships
Coordinate services among providers and care teams
Requirements:

Maintain Newfoundland and Labrador NP licensure
Maintain licensure in Alberta, Ontario, British Columbia, or Quebec
Have at least three years of NP experience
Demonstrate all-ages primary care experience
Communicate strongly and work independently
Bring independent practice, virtual care expertise, and patient focus to accessible primary care across participating provinces.
