Intake Social Worker Toronto

Support accessible community care as an Intake Social Worker with VHA Home HealthCare, using telephone assessment and client-centred planning. Serve adults, seniors, and families across Toronto.
This temporary full-time contract runs until June 25, 2027. You will receive service requests, assess eligibility, and connect clients with VHA programs or relevant community resources. The position requires strong intake assessment, case planning, documentation, and communication skills.
Key Responsibilities:

Conduct comprehensive telephone intake assessments
Process service requests within 24 hours
Develop service plans with clients and families
Provide referrals and eligibility information
Maintain accurate client records and follow-up
Requirements:

Bachelor’s degree in social work or related field
One to three years’ intake or social service experience
Strong assessment, formulation, and case planning skills
Knowledge of mental health, addictions, and family dynamics
Ability to manage high-volume telephone inquiries
Bring intake assessment, client-centred planning, and community resource knowledge to VHA’s responsive care services.
