Georgian Bay General Hospital in Midland, Ontario is seeking a Scope Nurse Navigator (0.8 FTE) to provide leadership in navigation, referrals, and care coordination across primary care and hospital programs.
As a central contact, you will optimize access, communication, and continuity of care for the North Simcoe region, participating in SCOPE clinical pathways and ongoing process improvements. RN with Ontario registration and 3+ years of acute/community care is preferred.
