Home Care Clinical Supervisor

Apply clinical assessment and care planning skills while supervising Home Service Workers across Toronto and the surrounding GTA community.
This Client Services Supervisor role supports clients receiving scheduled home care and responds to changing needs. You will work with Service Coordinators to implement care changes, guide staff, and maintain timely information sharing. Responsibilities also include family meetings, performance appraisals, training, and documentation.
Key Responsibilities:
Assess client health, functioning, support networks, and home environments
Establish and update client care plans
Provide direction during crises and urgent visits
Arrange family, staff, and team meetings
Complete performance reviews and required documentation
Requirements:
Regulated healthcare professional with current College registration
Experience with elderly populations or community-based care
Strong written and verbal English communication skills
Accurate documentation and attention to detail
Windows, MS Office, and word processing skills
Bring regulated healthcare expertise to home care supervision, client assessment, staff education, and coordinated service delivery in Toronto.
