St. Leonard's Community Services is seeking a full-time Housing Stability Worker for the Mental Health Justice – Supportive Housing Program.
You will provide participant-centered case management, assessment, crisis intervention, advocacy and life skills support to adults facing housing instability and mental health challenges. Duties include coordinating with healthcare, addiction supports, landlords and justice partners, developing individualized support plans, and ensuring safety of clients
