Community Health Services Manager

Coordinate Community Health Services with CarePartners, using nursing knowledge, case management, and coaching skills in Niagara’s home care setting.
This Personal Support Manager position supports high quality patient care through effective coordination and supervision. Working from the St. Catharine's office, you will provide employee support and conduct home visits within the Niagara Community. The full-time role includes Monday-to-Friday hours and rotational Branch Manager On-Call participation during evenings and weekends.
Key Responsibilities:
Provide field supervision and coaching to Community Support Workers
Maintain effective care plans independently or collaboratively
Apply healthcare policies and community service best practices
Monitor employee performance and provide active guidance
Support service promotion and community public relations
Requirements:
Registered Nurse or Registered Practical Nurse credential
College of Nurses of Ontario registration
Three to five years of job-related experience
Proven computer skills with scheduling systems
Vehicle availability and regular travel capability
