Provide clinical counselling and biopsychosocial assessments as a Social Worker with Providence Health Care. Support patients, families, and interdisciplinary teams.
This Social Worker supports an interdisciplinary program under the Patient Care Manager’s direction. Responsibilities include clinical counselling, comprehensive assessments, patient records, discharge planning, and community-resource consultation. You will also contribute to conferences, presentations, committees, research, and student supervision.
Key Responsibilities:
Provide therapeutic counselling to patients, families, and groups
Conduct comprehensive social work and psychosocial assessments
Maintain patient records, progress notes, and reports
Coordinate discharge plans with hospital and community services
Deliver training, presentations, and staff education
Requirements:
Master of Social Work from an accredited university
One year of recent related experience, or equivalent preparation
Current full registration with British Columbia College of Social Workers
Ability to collaborate within interdisciplinary care teams
Ability to prioritize workload and work independently
