ParaMed’s home health care services need a Registered Nurse skilled in assessment and patient records. This full-time role serves patients in their homes.
As a community Registered Nurse, you will make scheduled visits across various settings. Patients range from children to older adults, with differing care complexity. Your practice includes executing individualized plans, monitoring changes, and supporting comfort at home for as long as possible.
Key Responsibilities:
Conduct patient assessments using suitable techniques
Execute individualized care plans during scheduled visits
Monitor care plans continuously and respond appropriately
Engage patients through collaborative, trusting relationships
Document encounters and care plans professionally
Requirements:
Registration with CNO as a Registered Nurse
Good standing with the College of Nurses of Ontario
Driver’s license and vehicle access required
Flexibility and reliability in community nursing
Availability for after-hours on-call coverage
Use clinical assessment, documentation, and care coordination skills as a ParaMed Registered Nurse supporting patients across home settings.
