Geriatric Nutrition Care Dietitian

Support vulnerable adults through geriatric nutrition assessment, hydration risk management, and care planning in a part time dietitian role.
Extendicare seeks a Registered Dietitian to monitor residents and guide nutrition care within long term care. You will assess nutrition status, determine nutrition and hydration risks, provide counseling, and coordinate follow up. The position contributes to resident care conferences, Skin/Wound Care, Bowel Management, and Dysphagia Management teams.
Key Responsibilities:
Monitor residents with high and moderate nutrition risks
Coordinate nutrition monitoring delegated to the Nutrition Manager
Provide education for residents, caregivers, families, and decision makers
Participate in interdisciplinary committees and resident care teams
Advise Administration on cultural, religious, language, and nutrient needs
Requirements:
Approved university degree in Food and Nutrition
Registration with the applicable province's regulatory College of Dietitians
Dietitians of Canada and Gerontology Network membership
Knowledge of provincial food, health authority, and resident regulations
Current vulnerable sector, TB, liability insurance, and WSIB documentation
Bring gerontology-focused assessment, counseling, monitoring, and care coordination skills to Extendicare's resident-centered nutrition services.
