Community nursing with patient assessment and care planning defines this part-time Registered Practical Nurse position with ParaMed. Scheduled visits support patients in their homes.
As a Registered Practical Nurse, you will provide care across community settings for patients of all ages and varying complexity. You will work within your scope, with Registered Nurse support for more complex patients. This position supports comfort and continued home living.
Key Responsibilities:
Complete scheduled community visits within your scope of practice
Provide ethical and safe nursing care
Assess individual patient needs using appropriate techniques
Execute, monitor, and adjust care plans as required
Maintain professional records for patient encounters and plans
Requirements:
Registered Practical Nurse registration with CNO in good standing
Valid driver’s license and access to a vehicle
Willingness to travel between patient residences
Reliability and flexibility
Required after-hours on-call coverage
Support ParaMed patients at home through community nursing, assessment, care planning, and collaborative therapeutic relationships.
