Key Responsibilities:
Conduct scheduled visits in various patient care settings

Execute and continuously monitor patient care plans
Build rapport and trust through collaborative engagement
Maintain and update professional patient records
Requirements:
Registered with CNO in good standing
Valid driver’s license and access to a vehicle
Community care experience is an asset
Reliability and flexibility
Willingness to travel within a designated area
