Advanced Outpatient Mental Health Social Worker

Advanced mental health practice meets recovery-focused care at Community Mental Health Clinic - NW, supporting adults through assessment and treatment. Work independently within a multidisciplinary team at Foothills Professional Bldg.
As a Social Worker III, you will provide advanced assessment, provisional diagnosis, therapy, and psychosocial interventions. The clinic serves adults with moderate to severe addiction and mental health concerns. You will support treatment planning, case management, crisis intervention, consultation, education, referrals, and follow-up while helping clients transition to medical homes.
Key Responsibilities:
Conduct comprehensive mental health assessments and provisional diagnoses
Provide therapy and authorized psychosocial interventions
Deliver case management, crisis intervention, and client follow-up
Coordinate transitions between outpatient clinics and medical homes
Consult with patients, families, and interprofessional teams
Requirements:
Master's degree in Social Work with authorization
Active or eligible Alberta College registration
Three to five years of mental health experience
Extensive CBT and DBT knowledge and experience
Suicide training, risk assessment, and psychosocial intervention certification
