Acute Care Neuro Social Worker

Support acute neuro patients at Foothills Medical Centre as a Social Worker II using psychosocial assessment and clinical judgment. Work regular full time days.
As a Social Worker II, you will provide social work services within an interdisciplinary team at this acute care hospital. The current caseload is acute neuro, with potential coverage across units or services as required. Services include counselling, case management, resource-based interventions, and discharge planning.
Key Responsibilities:
Provide psychosocial assessment for patients and families
Deliver individual, group, and family treatment
Coordinate case management and transition planning
Conduct risk screening and counselling
Support patient and family centered care
Requirements:
Complete a bachelor's degree in Social Work
Hold or qualify for ACSW registration
Bring at least six months adult acute care experience
Demonstrate clinical judgment and critical thinking
Show effective communication and conflict resolution skills
