Senior Care Licensed Practical Nurse

Support residents’ dignity and well-being at AgeCare through compassionate nursing care, medication administration, and interdisciplinary collaboration.
As a Licensed Practical Nurse reporting to the Director of Care, you will deliver patient-focused care in a senior care setting. The position combines resident assessments, care-plan coordination, family education, and direct nursing responsibilities. Experience in senior or long-term care is preferred.
Key Responsibilities:
Collaborate with care teams using person-centered approaches
Coordinate care plans with physicians and service providers
Educate residents and families about care plans
Support resident move-ins, discharges, and transfers
Address concerns and ensure timely follow-up
Requirements:
Current registration as a Licensed Practical Nurse
Senior care or long-term care experience preferred
Gerontology, rehabilitation, or complex care background
Strong communication and critical-thinking abilities
Current Vulnerable Sector Check or willingness to obtain
Use resident-focused nursing, family communication, and care coordination skills to support quality service at AgeCare.
