Community Mental Health Case Manager
Stella's Circle Community Services Inc.
Support adults in St. John's through intensive case management, recovery-oriented practice, and harm reduction as a full-time Front Line Case Manager.
Stella's Circle seeks a Case Manager for its Community Support Program. You will develop individualized support plans, coordinate Community Mental Health Workers, and provide supportive counselling, system navigation, advocacy, and housing support. This permanent contract runs until November 30, 2029.
Key Responsibilities:
Complete assessments with participants, referral sources, and the CSP team
Develop and review individualized support plans with participants
Provide supportive counselling and crisis prevention using stated approaches
Coordinate housing support, eviction prevention, and service referrals
Maintain documentation, risk assessments, and participant case plans
Requirements:
Undergraduate degree in social work, psychology, nursing, or social sciences
At least five years supporting adults with mental health or substance use needs
Knowledge of community services, recovery, harm reduction, and social justice
Valid driver's license and access to a reliable vehicle
Vulnerable Sector Check and Reliability Status required
