Community Visiting Registered Nurse
ParaMed Home Health Care
Deliver community nursing through scheduled home visits as a Registered Nurse, using patient assessment and palliative care expertise. Support patients across all ages.
ParaMed’s full-time visiting nursing role serves patients in various settings and supports comfort at home. You will assess individualized needs, execute and monitor care plans, build trust, and maintain professional records. Experience in Pediatrics and Palliative care is required.
Key Responsibilities:
Assess individualized patient needs using appropriate assessment tools
Execute and continuously monitor individualized care plans
Build rapport and trust through collaborative patient engagement
Maintain and update professional patient records
Provide required after-hours on-call coverage for patient safety
Requirements:
Registration with CNO as a Registered Nurse in good standing
Experience in Pediatrics and Palliative care
Valid driver’s license and access to a vehicle
Reliability and flexibility for community nursing visits
Willingness to travel between patient residences
