Provide compassionate home care as a Registered Practical Nurse across HNHB boundaries, applying nursing assessment and care planning skills. This part-time position includes travel.
St. Joseph’s Home Care seeks a Registered Practical Nurse for its Integrated Comprehensive Care Program. The regular position supports clients in homes and alternative settings through visiting or shift care. You will follow nursing processes, SJHC practices, and College of Nurses of Ontario standards while supporting healing and independence.
Key Responsibilities:
Provide quality nursing care within the scope of practice
Complete comprehensive nursing assessments and documentation
Review, follow, and evaluate nursing care plans
Consult with clients, caregivers, teams, and external stakeholders
Conduct virtual care visits when required
Requirements:
Current unrestricted RPN registration with the College of Nurses of Ontario
Practical Nursing Diploma or Certificate from an accredited college
Required IV medication administration education or certificates
Basic Cardiac Life Support certification
Specific nursing care or mental health experience preferred
Bring registered practical nursing expertise, compassionate assessment, and care planning to clients across HNHB boundaries.
