Registered Practical Nurse ALC Facility

Registered Practical Nurses provide safe, compassionate care at Humber River Health’s Reactivation Care Centre, supporting general medicine, ALC, and seniors’ care.
This temporary full-time position uses rotating eight- and twelve-hour shifts, including days, nights, evenings, and weekends. RPNs coordinate care within interprofessional teams and complete electronic Meditech 6.0 documentation. The role supports Humber River Health’s newly designated Alternative Level of Care facility.
Key Responsibilities:
Provide RPN care for general medicine, ALC, and seniors
Coordinate patient care within an interprofessional team
Complete electronic documentation using Meditech 6.0
Participate in unit projects or hospital committees
Mentor new hires and pre-grad RPN students
Requirements:
Recognized two-year Registered Practical Nursing diploma
Current College of Nurses of Ontario registration
Current BCLS certification
Advanced physical assessment skills
Minimum two years’ recent medical or seniors’ care preferred
Bring clinical judgment, patient teaching, and nursing skills to Humber River Health’s ALC care team.
