Community Connector
Swift Current Ag
With funding from Saskatchewan Seniors Mechanism, the City of Swift Current is participating in an 18-month social prescribing pilot for older adults. The project funds a half-time contract to implement social prescribing in the community.
The Community Connector accepts referrals from health care providers and other sources and works one-on-one with older adults to support their health and well-being. Using a holistic approach, the role connects participants with community groups and services that provide practical and emotional support.
The Community Connector also strengthens community capacity by collaborating with community groups, the Saskatchewan Health Authority, and other health partners to support strong, inclusive local assessments and sustainable community connections.
KEY ACCOUNTABILTIES
Experience working directly with individuals, including case management and care planning.
Experience in community development, therapeutic recreation, adult health or social care, or public health.
Experience supporting older adults, families, and caregivers.
Experience collaborating across organizations.
Ability to maintain confidentiality and demonstrate professionalism, diplomacy, and discretion in sensitive situations.
Strong time-management skills, including balancing multiple priorities and managing grants.
Strong attention to detail and the ability to conduct research, collect data, and prepare and maintain accurate records and documentation.
Strong interpersonal, listening, and communication skills, with the ability to build trust and credibility at all levels.
Initiative, sound judgment, and problem-solving skills to address challenges effectively.
Ability to provide a clear criminal record check and vulnerable person check completed within the past month.
Willingness to work flexible hours to meet work demands.
Access to personal transportation and the ability to travel locally, including for home visits.
