Community Registered Practical Nurse

Deliver community nursing care as a Registered Practical Nurse with scheduled patient visits, clinical assessment, and support from Registered Nurses. Help patients remain comfortable at home.
ParaMed seeks a Registered Practical Nurse for scheduled visits across community settings. You will care for patients of all ages, assess individual needs, execute care plans, and build therapeutic relationships. Complex patients receive support from Registered Nurses, while you work within your scope of practice.
Key Responsibilities:
Complete scheduled community visits within designated areas
Provide ethical, safe nursing care within scope
Assess individual patient needs using appropriate tools
Monitor and adjust care plans as required
Maintain professional patient records after encounters
Requirements:
Current CNO registration as a Registered Practical Nurse
Valid driver’s license and access to a vehicle
Willingness to travel between patient residences
Reliability and flexibility for community nursing schedules
Required after-hours on-call coverage for patient safety
