Mental Health Case Coordinator Nurse

As a Mental Health and Addictions Case Coordinator Nurse, you provide treatment services through a psychosocial rehabilitation model. You work as an RN or RPN within a multidisciplinary team, applying counseling, crisis intervention, and treatment planning skills. Client and family relationships guide care coordination.
Key Responsibilities:
Conduct intake and mental health assessments
Develop treatment plans and provide crisis intervention
Provide outreach visits, counseling, and education
Coordinate client access across the care continuum
Facilitate support groups and multidisciplinary consultation
Requirements:
Diploma in Nursing or Psychiatric Nursing
Two years’ recent related mental health treatment experience
Current BCCNM registration as an RN or RPN
Valid British Columbia driver’s license
Knowledge of addictions treatment and psychopharmacology
