Mental Health Clinical Social Worker
St. Joseph's Healthcare Hamilton
Use clinical social work skills at St. Joseph’s Health System in Hamilton, supporting assessment, counselling, and case management. This weekday position runs on days.
As a Registered Social Worker, you will deliver services consistent with Ontario professional standards and evidence-based practice. The Forensic Psychiatry Program supports adult offenders with mental disorders through assessment, treatment, rehabilitation, and community reintegration. Your practice will help patients pursue recovery and quality of life while supporting timely, risk-reduced transitions.
Key Responsibilities:
Complete psychosocial assessments for mental health patients
Deliver individual, family, and group treatment modalities
Provide crisis intervention, counselling, and case management
Coordinate discharge planning and community services
Consult with inter-professional teams and support advocacy
Requirements:
Registered with the Ontario College of Social Workers
Bachelor’s degree in Social Work required
Master’s degree in Social Work preferred
Knowledge of mental health populations and screening tools
Ability to prioritize, meet deadlines, and work under pressure
