Health Promotion Outreach Worker

Health promotion and disease prevention are central to this in-person Community Outreach Worker position at Quest Community Health Centre.
Within the Urgent Service Access Team, the Community Outreach Worker delivers education, outreach, and social assistance services. The position supports community capacity building while addressing social determinants of health through a harm reduction framework. Work occurs within Quest CHC’s interdisciplinary primary health care model.
Key Responsibilities:

Promote health and support disease prevention initiatives
Provide outreach and client health education
Administer varied social assistance programs
Implement community services for client support
Build community capacity through collaborative practice
Requirements:

Availability for full-time employment
Availability for 37.5 hours each week
Ability to work on site
Understanding of harm reduction practice
Commitment to addressing social determinants of health
Apply community outreach, health education, and prevention skills within Quest CHC’s Urgent Service Access Team.
