Support clients at home as a Community Health Worker with CVHCC in Courtenay, using personal care and daily living assistance. Regular travel between sites is required.
As part of a multidisciplinary health care team, you will support clients and caregivers or families. The work promotes independence through established plans of care, delegated tasks, and personal assistance. This regular position includes evening hours from 1800 to 2200.
Key Responsibilities:

Provide personal care assistance in clients' homes
Support activities of daily living and delegated tasks
Follow established plans of care for assigned clients
Assist caregivers and families with client support
Travel regularly between multiple service sites
Requirements:

Recognized Home Support or Health Care Assistant certificate
Food Safe Certificate and valid British Columbia driver's license
Current good standing with BC Care Aide Registry
Ability to work independently with minimal supervision
Effective communication and physical ability to perform duties
Bring personal care, communication, and independent support skills to promote client independence across Courtenay home settings.
