Registered Practical Nurse Long Term Care

Provide professional nursing care as a Registered Practical Nurse with AgeCare, supporting residents through long-term care expertise and clinical communication.
This Registered Practical Nurse role delivers resident care according to medical requirements and professional nursing practice. You will collaborate with Registered Nurses, physicians, health care teams, and external partners. The position also provides direction to Personal Support Workers while supporting informed, coordinated resident care.
Key Responsibilities:
Provide professional nursing care to residents
Implement AgeCare policies and procedures with Registered Nurses
Liaise with physicians and health care partners
Notify physicians about significant resident status changes
Direct Personal Support Workers on the care team
Requirements:
Knowledge of geriatric, rehabilitation, or long-term care nursing
Understanding of activation, nursing, and personnel administration
Knowledge of applicable health care legislation and regulations
Current College of Nurses of Ontario Certificate of Competence
Membership in good standing with the College of Nurses of Ontario
Bring long-term care nursing knowledge, resident assessment, and team coordination skills to AgeCare’s professional nursing practice.
