Canadian Mental Health Association HKPR in Peterborough seeks Mental Health Housing Workers for the HART Hub. The role delivers community-based mental health and housing supports within a 24/7 program, requiring rotating shifts, overnights, and weekends.
You will work with a multidisciplinary team to assess needs, develop treatment goals, facilitate housing transitions, document plans, and coordinate care with partners. Preference for related degree and clinical training.
