Guide seniors through personalized care at Chartwell as a Health Navigator, using clinical assessment and compassionate communication. Collaborate with care teams in a retirement community.
As a Health Navigator, you will work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. The position involves collaboration with residents, families, physicians, and care teams. Your work supports personalized, high-quality care through nursing knowledge, documentation, and resident education.
Key Responsibilities:
Complete resident assessments and individualized care plans
Coordinate admissions, conferences, transfers, and discharges
Provide nursing care and medication administration
Partner with physicians, families, and care teams
Monitor wellness outcomes and clinical documentation
Requirements:
RPN, LPN, or RN registration in good standing
Current CPR and First Aid certification
Medication administration knowledge
Healthcare experience in retirement or related settings
Strong assessment, clinical, organizational, and communication skills
Bring compassionate nursing, assessment, and coordination skills to support Chartwell residents and their personalized health journeys.
