This Personal Support Worker position serves a roster of clients through a diverse, self-directed care team. You will contribute to client-centered planning across physical, cognitive, social, emotional, and spiritual domains. The team connects care with client goals and supports improved transitions home.
Key Responsibilities:

Serve a roster of community clients
Deliver services using a client-centered approach
Contribute to holistic care planning
Address physical and cognitive client needs
Support social, emotional, and spiritual goals
Requirements:

Valid Personal Support Worker certificate, RN, or RPN
Foreign-trained nurse healthcare professional accepted
Required immunizations, including MMR, Varicella, TB, Tetanus, Covid
Valid driver’s license and work vehicle
Physical ability to lift approximately fifty-five pounds
Use autonomy, collaboration, and holistic planning to support SE Health clients receiving care throughout their communities.
