Home Dialysis Registered Practical Nurse

Support home dialysis patients as a Registered Practical Nurse with peritoneal dialysis expertise across the community and Burlington. Rotating 7.5-hour shifts apply.
Halton Healthcare seeks an RPN for its Home Dialysis Assistance Program. You will provide assessment, education, decision support, and follow-up care for patients and families. The work includes home visits, dialysis management, medication administration, and collaboration with the renal interdisciplinary team.
Key Responsibilities:
Assess patients receiving home peritoneal dialysis
Educate patients and families about home dialysis
Coordinate appointments and community home visits
Prepare circuits and program dialysis machines
Monitor treatments and provide interventions with RN collaboration
Requirements:
Current College of Nurses of Ontario registration in good standing
Current BCLS certification at time of hire
Experience processing physician orders
Access to a vehicle for community transportation
Ability to practice fully within this environment
Bring nephrology nursing practice, patient education, and therapeutic support to Halton Healthcare’s community home dialysis program.
