Remote Advocacy Care Navigator

Support existing Sailor Health patients remotely across Canada through advocacy intake, care coordination, and compassionate communication. Help older adults access continued support.
As a full-time Advocacy Care Navigator, you will guide patients already working with therapists toward advocacy services. The work combines patient support, care coordination, and emotional guidance. You will provide continuity beyond therapy sessions through organized, attentive communication.
Key Responsibilities:

Conduct introductory calls with existing advocacy-service patients
Guide patients through advocacy onboarding with clarity
Coordinate transitions and follow-through with advocacy teams
Manage follow-ups, missed calls, and reassignments
Maintain accurate records in Google Workspace, Healthie, and HubSpot
Requirements:

Empathetic, clear communication during vulnerable conversations
Strong organization and attention to detail
Comfort navigating Google Workspace, EHRs, and CRMs
Commitment to improving care for older adults
Healthcare, customer service, or administrative experience welcomed
Bring empathy, organization, and technology comfort to advocacy navigation for Sailor Health patients across Canada.
