Community Care Registered Nurse

Community care Registered Nurses assess patients across all ages while supporting comfort, individualized needs, and continuity of care in patients’ residences.
ParaMed offers a new casual Registered Nurse position within its home health care services. You will assess patients, execute care plans, build therapeutic relationships, and maintain professional records. The work requires travel, flexibility, and on-call coverage.
Key Responsibilities:
Assess individualized patient needs using appropriate clinical tools
Execute and continuously monitor patient care plans
Build rapport and trust through collaborative patient engagement
Maintain and update professional patient encounter records
Provide required after-hours on-call coverage for patient safety
Requirements:
Registration with CNO as a Registered Nurse in good standing
Valid driver’s license and access to a vehicle
Community care delivery experience considered an asset
Reliability and flexibility for patient care scheduling
Willingness to travel within a designated area
Support ParaMed patients through assessment, care planning, therapeutic relationships, and reliable community-based nursing service.
