Social Worker Mental Health Clinician

Apply mental health assessment and counselling expertise as a Social Worker Mental Health Clinician with the Iroquois Falls Family Health Team.
This clinician supports patients experiencing mental health and psychosocial concerns within an interdisciplinary primary care team. The work includes individual counselling, therapeutic interventions, individualized care planning, and connections to additional services. Candidates bring relevant education, registration, and clinical competence.
Key Responsibilities:
Provide individual counselling and brief therapeutic interventions
Develop individualized treatment plans and monitor patient progress
Manage counselling caseloads and maintain timely documentation
Triage referrals and prioritize appointments by clinical urgency
Coordinate telemental services and community referrals
Requirements:
Bachelor’s or Master’s degree in a related mental health discipline
Current Ontario regulatory college registration in good standing
Experience in assessment, counselling, and therapeutic interventions
Knowledge of CBT and DBT, or willingness to pursue training
Electronic medical record and virtual care technology proficiency
Support coordinated patient care through mental health assessment, counselling, therapy, and service navigation in Iroquois Falls.
