Home Care Registered Nurse Erie St Clair

Home care nursing at ParaMed centers on patient comfort, individualized assessments, and scheduled visits across County Area, Erie St. Clair District. Care for patients of all ages and varying needs.
This full-time Registered Nurse position supports patients in their residences and other settings. You will use assessment techniques, execute and monitor care plans, and maintain professional records. Collaborative communication helps establish rapport, trust, and long-term therapeutic relationships with patients.
Key Responsibilities:
Conduct assessments of patients’ individualized care needs
Provide scheduled nursing visits in various settings
Execute care plans and monitor patient progress continuously
Engage patients through collaborative, trust-building approaches
Update records for encounters and patient care plans
Requirements:
Registered Nurse registration with the CNO in good standing
Valid driver’s license and access to a vehicle required
Willingness to travel between patient residences
Reliability and flexibility in community nursing work
Availability for required after-hours on-call coverage
Use your Registered Nurse expertise in assessment, monitoring, and patient engagement while delivering ParaMed home care across Erie St. Clair.
