As an experienced LPN, your responsibilities include implementing patient-oriented care in alignment with provincial standards. You'll collaborate closely with a professional team, ensuring all assessments, care plans, and documentation are current and effective. Engage in resident care conferences, assist with transitions, and provide direct care, including medication administration.
Key Responsibilities:
• Complete and assess resident clinical care documentation
• Implement and review resident care and service plans
• Participate in care conferences for effective communication
• Collaborate with the Director of Care for resident transitions
• Administer direct care and medications adhering to standards
Requirements:
• Practical Nurse Diploma and current registration
• Experience in senior or long-term care settings
• Current CPR and First Aid certifications
• Leadership, mentorship, and teamwork skills
• Willingness to complete a Vulnerable Sector Check
