Support patient navigation through referrals, follow-up activities, and responsive reception service across both practice sites. Use SystmOne to maintain accurate information.
As a Care Coordinator/Receptionist, you will help patients access appropriate health and care services. You will coordinate appointments, referrals, recalls, and follow-up arrangements. The position also involves liaising with carers, clinicians, care homes, and external healthcare providers when appropriate.
Key Responsibilities:
Help patients access appropriate health and care services
Liaise with carers, clinicians, care homes, and providers
Monitor outstanding tasks and follow up appropriately
Maintain accurate records of actions and arrangements
Identify delays and escalate issues requiring attention
Requirements:
Ability to support patients requiring additional assistance
Collaborative approach with multidisciplinary team members
Accurate data entry and record updates
Understanding of relevant practice processes
Ability to recognise role limits and seek guidance
Apply patient coordination, referral management, and careful recordkeeping to help deliver timely, effective healthcare services.
