The Interlake-Eastern Regional Health Authority seeks a Home Care Case Coordinator for its non-clinical Home Care department. This position coordinates professional and non-professional services through a client-driven process. It assesses, plans, implements, monitors, and evaluates options meeting health and human service needs.
Key Responsibilities:
Coordinate professional and non-professional home care services
Assess client health and human service needs
Plan and implement appropriate service options
Monitor and evaluate client services and resources
Liaise with agencies, facilities, clients, and family caregivers
Requirements:
Required BSW, OT, PT, RPN, RN, LPN, RD, or related degree
Required professional licensure or registration in good standing
Preferred two years’ community-based care experience
Experience with elderly and palliative care clients
Valid Manitoba driver’s licence and vehicle required
