Field Nurse Case Manager Alberta

Support patient care across Alberta as a Field Nurse Case Manager using clinical education and therapy coordination. Work is hybrid, remote, and travel-based.
At McKesson, this nursing role supports patients within a Patient Support Program. Provide case management, treatment education, adherence support, and healthcare navigation while collaborating with providers, pharmacies, and reimbursement teams. Help facilitate therapy access and patient outcomes.
Key Responsibilities:
Deliver patient-centric case management within program protocols
Provide therapy education, onboarding, adherence support, and follow-up
Coordinate care with physicians, clinics, pharmacies, and internal teams
Support funding, prior authorizations, and patient access solutions
Document interactions and report adverse events appropriately
Requirements:
Diploma or degree in Nursing
Current unrestricted Alberta nursing registration in good standing
Experience providing patient care, education, coordination, or case management
Strong English communication and Microsoft Office proficiency
Valid driver's license and reliable transportation
