Senior Care Case Manager
Greater Springfield Senior Services, Inc
Support geriatric members through case management, bilingual communication, and hybrid work three days weekly after training is complete.
As a Geriatric Support Services Coordinator, you will function within the Primary Care Team for members in a Senior Care Option plan. Coordinate contracted and community services while monitoring health, functional status, wellness, and independence. Conduct reassessments and connect members with public benefits.
Key Responsibilities:
Function as a case manager within the Primary Care Team
Make recommendations for contracted and community services
Coordinate and implement member service plans
Monitor health and functional status
Conduct periodic reassessments of service plan appropriateness
Requirements:
BA or BS in Social Work, Human Services, or related field
Case management experience with geriatric populations
Spanish and English fluency
Valid driver’s license or reliable transportation
LSW is a plus
