Hospital Social Worker Perinatal Care

Provide perinatal psychosocial care at Humber River Health’s Wilson Site, using communication, advocacy, and community-resource planning skills.
This part-time hospital Social Worker serves the Maternal & Child team within a multi-site healthcare organization. The position supports crisis intervention, patient and family counseling, discharge planning, and community connections. It also contributes to committees, policy development, staff direction, and statistical reporting.
Key Responsibilities:
Connect patients with appropriate community resources
Support discharge planning and family-centered care
Contribute to committee work and policy development
Compile statistical reports supporting program improvement
Direct and educate staff about psychosocial care
Requirements:
Master’s degree in Social Work from an accredited program
Eligibility for OASW and OCSWSSW membership required
Recent obstetrical and paediatric experience required
Excellent organizational, interpersonal, and written communication skills
Knowledge of diverse community needs and equity principles
Use social work expertise, community planning, and perinatal knowledge to strengthen patient-centered care at Humber River Health.
