Complex Health Care Coordinator

Coordinate care for service users with complex health presentations through assessment, care planning, and specialist professional advice. This countywide role requires independent driving.
As Care Coordinator with Cambridgeshire and Peterborough NHS Foundation Trust, you will assess individuals, families, and carers accessing the service. The work includes supporting service users presenting with higher levels of risk and developing care plans alongside them. You will collaborate with service users and carers while managing and reviewing risks.
Key Responsibilities:

Undertake assessments of complex health presentations
Identify higher levels of risk during assessments
Develop care plans with service users
Provide specialist professional assessment and advice
Review and change risk management as necessary
Requirements:

Hold a full UK driving licence
Have use of a vehicle
Travel independently throughout the county
Meet strict time deadlines while travelling
Recognise public transport cannot meet service needs
Use assessment, care planning, and risk management expertise to coordinate support for complex presentations across the county.
