This part-time community nursing position serves patients across home, clinic, retirement home, and school settings. You will develop Care Plans independently or with Ontario Health atHome. The work includes weekday, weekend, and on-call hours within a supportive, collaborative care environment.

Provide exceptional nursing services to patients
Create Care Plans for Community Health Services
Work independently or with Ontario Health atHome
Communicate with families and interdisciplinary care teams
Perform clinical duties as required
Requirements:

Registered Nurse degree or diploma
Good-standing registration with the College of Nurses of Ontario
Strong commitment to improving patient lives
Valid driver’s license and personal vehicle
Current CPR, immunization, and vulnerable sector checks
Bring registered nursing expertise, Care Plan development, and clear communication to varied community care settings across Lambton County.
