Key Responsibilities:
• Provide complete nursing assessments focusing on holistic health
• Perform patient triage in person and via phone
• Collaborate to develop therapeutic treatment plans
• Offer health education and counseling to improve wellness
• Refer patients to hospital and community resources
Requirements:
• Bachelor of Science in Nursing required
• Registration from the College of Nurses of Ontario
• 3-5 years of nursing experience needed
• BCLS certification mandatory
• Understanding of primary care for diverse populations
