Complex chronic care, therapeutic interventions, and health counseling shape this Registered Nurse assignment at Fraser Health’s Community South Fraser in Surrey, B.C.
The relief full time position supports clients with chronic health conditions through individualized assessment and evidence-based nursing care. You will provide education, self-management support, and direct care while coordinating services with families, providers, and community agencies. The assignment is available until approximately April 2027 or the incumbent’s return.
Key Responsibilities:
Provide clinical care and comprehensive explanations to clients
Teach self-management, self-monitoring, wellness, and health promotion
Delegate and monitor client-specific tasks for Community Health Workers
Facilitate care conferences and evaluate client care plans
Participate in quality improvement, education, and professional practice activities
Requirements:
Current Registered Nurse registration with BCCNM
One year recent clinical experience in acute or community care
Experience with care and discharge planning or equivalent qualifications
Knowledge of community resources and continuum-of-care services
Physical ability to perform the position’s duties
Use clinical assessment, chronic care expertise, and health counseling to support coordinated client care with Fraser Health in Surrey.
