Deliver hospital-to-community outreach and counselling in Abbotsford, B.C., supporting children and adolescents with mental health and substance use concerns.
Fraser Health is hiring a Casual Hospital Community Outreach Counsellor, CYYA, within a multidisciplinary Transition Team. You will provide life skills training, coaching, advocacy, and support to clients and caregivers. Care is delivered through established treatment plans, trauma-informed approaches, and evidence-informed practice.
Key Responsibilities:
Provide life skills training, coaching, advocacy, and support
Use psycho-education, de-escalation, and behaviour management techniques
Assist with home visits, groups, outreach, and client meetings
Support appropriate medication management under established treatment plans
Facilitate attendance at appointments, programs, and services
Requirements:
Baccalaureate degree in an appropriate health-related discipline
One year recent multidisciplinary experience with youth mental health
Demonstrated ability to support mental health or substance use needs
Knowledge of behaviour management strategies and techniques
Ability to communicate effectively verbally and in writing
