Registered Practical Nurse Care Coordinator

Coordinate individualized client care with SE Health as a Registered Practical Nurse, using care plans, collaboration, and evidence-based practice. Flexible schedules support varied availability.
This nursing position centers on comprehensive assessments and personalized care planning. You will implement and evaluate care, serve as a point-of-care lead when designated, and collaborate with community services during transitions. SE Health supports development through training, education bursaries, tuition support, and mentorship.
Key Responsibilities:
Assess client needs comprehensively and develop individualized plans
Implement care using current evidence and best practices
Evaluate care outcomes against each client’s needs
Serve as point-of-care lead for designated caseloads
Support transitions through community-service connections
Requirements:
Certificate of Competence with the College of Nurses of Ontario
Availability for rotating days, evenings, nights, and weekends
Updated immunization records upon hire
Criminal background and Vulnerable Sector checks upon hire
Current CPR and required SE Health certifications
Use registered practical nursing, client assessment, and care coordination skills to strengthen individualized support through SE Health.
