Community Hospice Registered Nurse
Suncrest Hospice
Support exceptional patient-centered hospice care with Brighton/Suncrest Hospice as a Registered Nurse serving communities throughout the service area. Strong organization matters.
As a Registered Nurse, you will provide compassionate care across private residences, skilled nursing facilities, and assisted living communities. You will collaborate with interdisciplinary clinicians to create individualized plans. The position requires time management, EMR familiarity, reliable transportation, and the ability to manage visits, meetings, documentation, and urgent needs independently.
Key Responsibilities:
Provide individualized hospice care in community settings
Collaborate with nurses and interdisciplinary care professionals
Develop plans supporting patients and their families
Balance patient visits, meetings, documentation, and urgent needs
Build meaningful relationships through compassionate personalized care
Requirements:
Registered Nurse license in good standing in practice state
Minimum one year of hospice experience
Passion for exceptional patient-centered hospice care
Excellent organizational and time management skills
Comfort learning and using electronic medical records
