Clinical Social Worker Renal Program

Support renal clients through clinical counselling, treatment planning, and education as a Casual Social Worker at Surrey Memorial Hospital. Work within Fraser Health’s interdisciplinary Renal Program.
Reporting clinically to the Professional Practice Chief, Social Work, this position provides psycho-social treatment for clients and families. Responsibilities include addressing illness adjustment, lifestyle changes, bereavement, treatment objectives, and access to practical resources. The role also contributes to comprehensive care planning and program activities.
Key Responsibilities:
Assess social, emotional, cultural, financial, and environmental situations
Provide counselling about illness adjustment and personal concerns
Deliver education about treatment and lifestyle implications
Advise clients, families, and staff about available resources
Participate in comprehensive interdisciplinary care planning
Requirements:
Bachelor’s degree in Social Work
Two years’ recent clinical experience in healthcare settings
Full registration with the BC College of Social Workers
Ability to apply systems theory to social work practice
Ability to organize and prioritize work
Use clinical social work expertise, systems theory, and counselling skills to support renal clients and families in Surrey.
