Indigenous Health Liaison & Care Navigator

Fraser Health Authority in British Columbia is seeking an Indigenous Health Liaison Social Worker to join a multi-disciplinary team across hospital and community settings. You will provide culturally appropriate counselling and intermediary services to First Nations, Métis and Inuit clients, planning and coordinating care with other programs and Indigenous partners.
